Qualifications

• Collects patient/family data to identify clinical, educational, psychosocial, and cultural needs and communicates observations to the health team.
• Contributes to the patient plan of care based on clinical, educational, psychosocial needs and coordinates with the team.
• Plans, prioritizes and organizes patient care tasks to ensure time and resource utilization.
• Performs routine patient care tasks and medication administration; intervenes in emergencies when needed.
• Personalizes nursing interventions to patient and family needs and adjusts as care changes.
• Reassesses and adjusts the care plan with the RN.
• Performs other duties as assigned.
