The role focuses on coordinating care, educating families, and preventing ER visits, often working behind the scenes to avoid hospital visits. Schedule is Monday–Friday, 8 AM to 5 PM, with a supportive environment and flexible scheduling.

Qualifications

• Minimum of 2 years RN experience in clinic or hospital.
• Experience with patients who have complex liver diseases or gastro issues preferred.
• No New Grads.
• Ability to educate families to prevent ER visits.
• Strong coordination of care and collaboration with the healthcare team.

Responsibilities

• Perform comprehensive nursing assessments across physical, psychological, social and educational factors.
• Implement and monitor plans of care in line with regulatory policies and guidelines.
• Coordinate care transitions and discharge planning within the organization.
• Provide health education for health promotion and disease management.
• Advocate for patients and families and support navigation of ambulatory services.
• Document in the medical record and ensure patient safety.
