Job summary

Qualifications

• Minimum of 2 years nursing experience in a clinic and/or hospital environment.

Responsibilities

• Utilizes multiple platforms to complete a nursing assessment of patients and families across age groups and conditions.
• Implements plans of care, provides health education for health promotion and management of conditions.
• Coordinates care and manages transitions within the organization and community, collaborating with the healthcare team.
• Advocates for patients and families, guiding ambulatory service navigation and resource management.
• Continuously evaluates patient understanding and response to the plan of care; documents in the chart.
• Contributes to patient safety and quality initiatives; supervises assigned team members and provides feedback.
• Mentors students and new employees, supporting clinical instruction and knowledge sharing.
• Performs miscellaneous duties as requested.
