Qualifications

• Minimum 2 years ICU experience.
• Graduate of an NLN accredited school of nursing.

Responsibilities

• Utilizes appropriate assessment skills to determine patient needs.
• Develops a plan of care for those patients whose needs do not require the expertise of a Case Manager or collaborates with the Case Manager to develop a plan of care for specified patients.
• Provides input into the development of clinical paths with assigned Case Manager.
• Delivers patient care using care plans and clinical paths and delegates as appropriate based on patient needs and staff competencies.
• Documents appropriately in patient care records.
• Prepares and administers medication/treatment according to physician orders, ensuring patient safety.
• Performs in collaboration with the multidisciplinary team to provide patient centered care.
• Incorporates discharge planning into the care plan to ensure smooth transition from hospital to home.
