Connecticut Children's Medical Center in Hartford, CT seeks a Behavioral Health Family Support Clinician, LCSW- Per Diem to assess psychosocial and psychiatric needs and provide crisis intervention. You will coordinate care within the hospital team and with community resources to support families and patients.

The role requires an LCSW and at least 2 years of experience with children and families, with strong counseling and communication skills. Per diem schedule.

Formación

• Master's in social work is required.
• LCSW license required with ability to meet internal credentialing.
• Minimum of 2 years related experience with children and families.

Responsabilidades

• Assess psychosocial and psychiatric needs of patients and families.
• Develop therapeutic plans and provide crisis intervention.
• Coordinate care with multidisciplinary team and community resources.
• Advocate, refer and discharge planning to meet patient/family needs.
• Collaborate with payors and partners to ensure appropriate care.
• Participate in performance improvement and learning activities.
