Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Bachelor’s degree preferred; additional certifications may be preferred.

Responsibilities

• Assess patient physical, psychological, social, spiritual, educational, developmental, and discharge planning needs.
• Formulate a plan of care, including discharge planning, with patient/family input.
• Implement and coordinate the plan of care with the health care team; delegate appropriately.
• Evaluate patient progress and adjust the plan based on response to care and teaching.
• Document assessment, planning, implementation and evaluation in the patient record; ensure accuracy and timeliness.
• Contribute to quality improvement and support education and mentoring of staff.
