Qualifications

• Master's Degree in Social Work required.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.
• MI social work licensure required upon hire.
• Driver's license required upon hire (MI).

Responsibilities

• Coordinate care management and behavioral health interventions.
• Collaborate with patients and families to facilitate understanding of diagnoses and treatment plans.
• Respond to crisis situations and determine appropriate level of care across the continuum.
• Partner with community agencies to coordinate care and resources for patients and families.
