Responsibilities

• Conduct screenings and family assessments as part of the multidisciplinary team.
• Perform case management activities and coordinate services for eligible children.
• Update IFSPs and monitor timelines for service delivery and transition.
• Recommends and refers to community resources to meet identified outcomes on IFSPs.
• Maintain current and accurate client records for payment and compliance.
