Job summary

Powerback Rehabilitation to You in Skokie, IL seeks a licensed Physical Therapist to deliver early intervention services for children birth to three. You will perform evaluations, create and implement an IFSP-based plan, and document progress in collaboration with families and caregivers.

Qualifications

• Must have a valid Physical Therapy license in IL or be eligible.
• Must have a qualifying Physical Therapy degree.
• Early Intervention Temporary or Full Credential.
• Effective written and verbal communication skills.
• Proven leadership and customer relationship skills.
• Ability to analyze and solve problems with initiative.

Responsibilities

• Provides physical therapy services to infants/toddlers birth to three.
• Performs evaluations and updates IFSP documentation.
• Establishes treatment schedule and delivers services per IFSP.
• Participates in family and caregiver education.
