Job summary

Fair Haven Community Health Care seeks a DBP Clinical Coordinator to manage referrals, schedule visits, and serve as medical assistant during DBP clinic hours, ensuring smooth patient flow through the program.

You will greet patients, perform age-appropriate vital signs and point‑of‑care testing, prepare patient-facing letters, and support the team with administrative tasks while maintaining regulatory compliance.

Qualifications

  • High school diploma or GED required; associates degree preferred.
  • Bilingual English and Spanish preferred.
  • Minimum six months of medical facility experience.
  • Basic Life Support (BLS) certification required.
  • Certified medical assistant with diploma or CMA with at least three years of experience.

Responsibilities

  • Receive referrals to the DBP program and schedule patients for initial consultations under guidance of the Director of Behavioral Health Operations.
  • Coordinate intake processes for patients entering DBP care.
  • Work with DBP clinical staff in quality improvement initiatives.
  • Schedule follow-up appointments for patients in the DBP program.
  • Manage phone calls, refill requests, and other patient-originating messages to the DBP clinic team.
  • During DBP clinical hours, greet patients and families while creating a welcoming environment.
  • Listen to and determine patients’ needs in an efficient and friendly manner.
  • As needed, perform age-appropriate vital signs and point-of-care testing.
  • Prepare letters, flyers, and memos that are patient-facing.
  • Ensure compliance with current healthcare regulations, medical laws, and ethical standards.
  • Maintain competency in medical assistant tasks as overseen by the Director of Nursing.
  • Perform other necessary duties to provide excellent primary health care.
