Responsibilities

Patient Care – perform history and/or physical exams and diagnose illnesses; develop and monitor treatment plans; order and interpret tests; provide clinical consultation and referrals.
Outcomes Identification and Planning – develop measurable outcomes and an individualized care plan; involve patient/family when possible; adjust plans as needed.
Professional Practice – maintain professional behaviors, participate in quality improvement, mentor others, contribute to scholarly activity and policy influence.
Care Management – ensure comprehensive care; promote self-management and family-centered care; coordinate with care team and make appropriate referrals.
Evaluation – continuous evaluation of diagnoses and care plans; document outcomes and adjust as needed.
