Qualifications

• Doctor of Pharmacy degree required; Bachelor’s accepted if licensed before 2004.
• State of Ohio licensure or licensure-eligibility to practice.
• BPS Board Certification or eligible within 24 months of hire.

Responsabilités

• Provide pharmacotherapy guidance and evidence-based recommendations.
• Document patient care outcomes in the medical record and ensure timely medication delivery.
• Maintain accurate medication lists and communicate with care teams and families.
• Precept and supervise pharmacy residents, students, interns, or graduates awaiting licensure.
• Participate in hospital committees and quality improvement/safety initiatives.
