Responsibilities

• Provide nursing triage, assessment, and patient education through telephone encounters and secure messaging.
• Plan, deliver, and document evidence-based nursing care with supervision of the Nurse Manager and providers.
• Coordinate ABPM and holter monitor tracking, including scheduling, education, and data preparation for interpretation.
• Provide cross-coverage and support to ensure continuity of patient care and clinic operations.
• Participate in performance improvement initiatives and department meetings.
