Job summary

Children’s Healthcare of Atlanta is seeking a registered nurse to provide direct patient care using the nursing process within an interdisciplinary team. You will engage in care coordination, clinical guideline implementation, and telehealth support to aid patients and families.

The role emphasizes strong communication, continuity of care across clinic and home, and adherence to department-specific requirements such as licensure and BLS/PALS/PEARS timelines.

Qualifications

• 12 months RN experience required.
• Graduation from accredited school of nursing.
• BSN preferred.
• Licensure as Registered Nurse in GA or Multi-State via eNLC/BLS within 30 days of employment.
• PALS/PEARS within one year of employment as required by department.
• BLS within 30 days of employment.

Responsibilities

• Cultivates and maintains effective interaction with medical staff to drive care coordination.
• Communicates with multidisciplinary team to facilitate care coordination and plan of care.
• Coordinates with home health agencies and other resources as needed.
• Provides ongoing communication with patient/family to identify barriers and ensure continuity of care.
• Facilitates and applies clinical practice guidelines at the patient level.
• Performs outpatient and clinic care coordination and tracks patient care between clinic and home.
• Provides telehealth care to assist patient needs and concerns.
• Refers risk/quality issues to appropriate personnel.
