• Strong computer skills (EMR experience a plus).

Responsibilities

• Deliver exceptional service—ensure clients receive outstanding home health care with compassion and reliability.
• Manage and coordinate care—oversee scheduling, client needs, and service coordination.
• Build relationships—develop strong connections with clients, caregivers, referral sources, and community partners.
• Grow and expand—manage and proactively increase your caseload to help more clients in need.
• Collaborate and lead—partner with Clinical Managers to provide support and supervision to field employees.
• Monitor performance—oversee key metrics and ensure efficient fiscal management of your caseload.
