Job summary

Full Circle Health is seeking a Pediatrics Case Coordinator to coordinate comprehensive, family‑centered care for children and adolescents with complex health and social needs. You will work with families, providers, schools, and community partners to develop individualized care plans and improve health outcomes.

The role requires RN/LPN or MSW eligibility in Idaho, with two years of clinical practice; population health or transitions of care experience is preferred.

Qualifications

• Two+ years of clinical practice required; population health or case management experience preferred.
• Pediatrics experience preferred but not required.
• Valid license to practice RN or LPN or Master's Prepared Social Worker in Idaho; current BCLS; Certified Case Manager preferred.

Full Circle Health is looking for a Pediatrics Case Coordinator to join our team!PurposeThe Pediatric Case Coordinator coordinates comprehensive, family-centered care for children and adolescents with complex health and social needs. Working collaboratively with patients, families, providers, schools, and community partners, this role develops and manages individualized care plans, facilitates access to healthcare and community resources, addresses barriers to care, and supports care transitions to improve health outcomes, continuity of care, and overall well-being.
