Job summary

OU Health in Oklahoma City is seeking a Pediatric Care Management RN - PRN to coordinate care for select patient populations and optimize resource utilization.

You will assess, plan, implement, and evaluate care with the interdisciplinary team, educating families and interfacing with providers and payors to ensure timely services.

This PRN role requires minimum 2 shifts per month and a current RN license with BLS.

Qualifications

• Education: Associate’s degree in nursing required.
• Experience: At least 1 year of care management experience.
• License/Certification: RN license in OK or compact; BLS required.

Responsibilities

• Conduct comprehensive assessments of patients' health status, medical history, and ongoing care needs.
• Develop individualized care plans with the interdisciplinary team to ensure continuity of care.
• Educate patients and families on health conditions, treatment plans, medications, and self-care.
• Coordinate communication between patients, families, providers, and community resources.
• Evaluate and adjust care plans to improve outcomes and cost-effectiveness.
• Ensure timely authorization of services with insurance providers and payors.
