Responsibilities

• Documents assessments, interventions, plans and evaluations in accordance with regulatory requirements and SHC policy.
• Coordinates the plan of care to achieve outcomes and integrates work of the health care team.
• Advocates by supporting or recommending services to protect patient health, safety and rights.
• Collaborates with patients, families, caregivers and the health care team to ensure optimal outcomes.
• Arranges services among community agencies, physicians, patients, families and others involved in the plan of care.
