Responsibilities

• Leads outreach to members by phone or home visits to discuss care plans and educate the member and family.
• Coordinates activities based on care plans with healthcare and community providers and members/caregivers.
• Provides support and responds to complex member or provider inquiries related to care plans.
• Develops advanced knowledge of care management services for escalated issues.
• Communicates with care managers and providers to facilitate member services and continuity of care.
• Documents and reviews member records in compliance with state and regulatory requirements.
