Pediatric Care Coordinator - Bilingual Spanish or Mandarin-Cantonese Fluency

Care Coordinators are responsible for performing care management services to support members in navigating their healthcare system. The Care Coordinator collaborates with various teams to provide excellent customer service to NorthStar Members.

This position is 100% Remote.

Duties and Responsibilities
  1. Handle incoming calls from members regarding benefit questions, care coordination, program inquiries, connecting to their care manager, and screening assessments to identify medical intervention needs.
  2. Accurately document calls in the designated PHI system.
  3. Manage an assigned caseload requiring short-term care coordination and health navigation.
  4. Work within the interdisciplinary care team to communicate member issues and monitor screening to improve outcomes.
  5. Perform outreach calls to confirm services are in place.
  6. Perform additional duties as assigned.
Minimum Qualifications
  1. Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training/experience.
  2. High School diploma/GED.
  3. Effective verbal and written communication skills, with good grammar, spelling, and punctuation.
  4. Experience in a fast-paced environment handling multiple priorities.
Preferred Qualifications
  1. Associate degree.
  2. One year experience working with medically fragile children in the community.
  3. Experience in member outreach in a member services or call center environment.
  4. Healthcare background or familiarity with medical terminology, Medicaid, and Child Health Plus benefits.
  5. Bilingual in English and Spanish or Mandarin-Cantonese preferred.
Hiring Range
  • Greater New York City Area (NY, NJ, CT residents): $47,403 - $62,400
  • Other Locations within approved areas: $41,101 - $60,320

Salary and benefits depend on experience, education, licenses, and certifications. Benefits include medical, dental, vision, incentive programs, life insurance, and 401k. The hiring range reflects the expected salary range for new hires or transfers.
