AdventHealth in Orlando, FL is seeking an RN Care Coordinator to support Pediatric Specialty Programs, including multidisciplinary/surgery clinics. This full-time, in-person role uses community resources and coordinates care across teams to ensure smooth patient experiences from admission to discharge.

You will demonstrate nursing competency, provide patient and family education, and collaborate with surgeons, physicians, and staff to improve outcomes.

Qualifications

• Associate's degree in nursing required; RN license and BLS required.
• Bachelor's degree is preferred.
• Experience in pediatric care and case coordination is preferred.

Responsibilities

• Coordinate care for pediatric specialty programs and clinics.
• Act as liaison among families, home health, and medical staff.
• Promote evidence-based practice and quality improvement.
• Educate patients and families and ensure continuity of care.
