Avance Care in Charlotte, NC is seeking a Behavioral Health Therapist to join a growing, patient-centered primary care network. This full-time, hybrid role emphasizes collaborative care with onsite primary care providers and nutritionists to deliver integrated mental health services.

Qualified candidates hold a Master’s in a mental health field and NC licensure (LCSW/LCSW-A, LCMHC/LCMHC-A, LMFT/LMFT-A, or LPA) with 1+ year post-licensure experience, and a commitment to diversity and preventive

Qualifications

• Master’s degree in a mental health field with active NC licensure under LCSW/LCSW-A, LCMHC/LCMHC-A, LMFT/LMFT-A, or LPA.
• 1+ year post-licensure clinical counseling experience.
• Experience in integrated primary care or collaborative healthcare settings preferred.
• Commitment to diversity, inclusion and preventive care.

Responsibilities

• Provide outpatient clinical assessments and psychotherapy.
• Deliver individual or group therapy.
• Collaborate with onsite medical team to deliver holistic care.
• Coordinate care and warm handoffs with psychiatry and primary care teams.
• Utilize EHR for charting and scheduling.
