Job summary

Children’s Healthcare of Atlanta is seeking a social worker to provide high-quality psychosocial services, supporting patients and families across the continuum of care in a hospital setting.

Primary responsibilities include psychosocial assessments, discharge planning, resource coordination, and safety reporting within an interdisciplinary team. A Master’s in Social Work and LMSW licensure are required or preferred.

Qualifications

• Master’s degree in social work from an accredited graduate school.
• Georgia LMSW licensure is required or preferred.
• Experience in medical settings and with children is a plus.

Responsibilities

• Completes comprehensive psychosocial assessments to identify strengths and barriers across care.
• Assists patients and families in coping with illness, trauma, hospitalization, and end of life.
• Assists with discharge planning and ongoing medical care related to psychosocial needs.
• Helps families access resources to maintain stabilization and self-sufficiency.
• Assesses safety, abuse, and neglect concerns and reports to authorities as needed.
• Provides liaison for child protection, custody, and safety needs with teams.
• Documents and maintains updated patient information for services rendered.
