Job summary

Children’s Healthcare of Atlanta is seeking a licensed social worker to provide high-quality psychosocial services within our pediatric care teams. The role focuses on coordinating psychosocial needs for patients and families, including assessing strengths, identifying barriers, and supporting discharge planning.

Responsibilities include collaborating with interdisciplinary teams, guiding families through illness-related challenges, and ensuring access to community resources.

Qualifications

• Three years of postgraduate social work experience.
• Must hold a Master’s degree in social work from an accredited graduate school.
• Georgia Licensed Clinical Social Worker (LCSW) preferred or required.

Responsibilities

• Completes comprehensive psychosocial assessments to identify strengths and barriers across the continuum of care.
• Assists patients and families in coping with illness, trauma, hospitalization, and end of life.
• Assists families with discharge planning and ongoing medical care related to psychosocial needs.
• Helps families access resources to achieve stabilization and self-sufficiency.
• Identifies safety concerns and acts as mandated reporter for protections services.
• Liaises on child protection, custody, and safety needs with teams and community organizations.
• Maintains updated patient information related to services provided.
