Tri-Med Family Care seeks a Behavioral Health Case Manager to coordinate care for children and families within a pediatric primary care setting. Training occurs at a nearby site to support a smooth transition to the Dalton location.

You will work with the care team to identify needs, develop individualized care plans, and connect patients with appropriate community resources while maintaining professional boundaries.

Qualificações

• Ability to read and apply behavioral health information.
• Strong communication skills for interactions with families and providers.
• Ability to coordinate services across schools, providers, and community resources.

Responsabilidades

• Coordinate care plans and connect patients with resources.
• Identify patient needs, goals, and barriers with the clinical team.
• Maintain accurate records of contacts, referrals, and services.
• Support operations of Behavioral Health and Pediatric programs as needed.
