Cook Children’s in Prosper, TX seeks a Clinical Ambulatory Pharmacist to join the pediatric ambulatory care team. You will educate patients and caregivers, collaborate with multidisciplinary providers, and support medication access and specialty coordination across multiple clinics.
This role focuses on optimizing medication therapy, patient education, and care coordination within ambulatory pediatric clinics, with a schedule Monday-Friday 8a-5p CST.

SCHEDULE: Monday-Friday, 8a-5p CST (shift could vary slightly) *This position is supporting a clinical setting that includes but is not limited to face-to-face patient interaction, education and support. The Clinical Ambulatory Pharmacist serves as an integral member of the pediatric ambulatory care team, collaborating closely with providers, patients, caregivers, and pharmacy staff to optimize medication therapy outcomes. This role focuses on medication management, patient education, specialty medication access, and care coordination across multiple pediatric specialty clinics. This position is assigned to multiple pediatric specialty clinics and offers the opportunity to build strong collaborative relationships with providers, patients, caregivers, outpatient pharmacy teams, and specialty pharmacy services to improve medication access, education, and overall patient care.
Graduate from an American Council on Pharmacy Education accredited school of pharmacy PGY1 residency accredited by the American Society for Health-System Pharmacy or 2 years commensurate pharmacist experience in a health-system setting
Texas Pharmacist License or eligible for Texas Licensure as a registered pharmacist Pharmacist and pharmacy preceptor licensure must be obtained within 90 days of employment Current Basic Life Support for Healthcare Providers certification. Pediatric Advanced Life Support certification within 60 days of hire (critical care areas only)
