Qualifications

Responsibilities

• Provides direct patient care using the nursing process.
• Collaborates with interdisciplinary healthcare team to coordinate care.
• Communicates with team, home health, payors, and families to align care.
• Maintains clinical practice guidelines at patient level and supports care transitions.
• Coordinates outpatient and clinic care and telehealth interventions.
• Supports quality and safety initiatives and ongoing professional development.
