Crossing-Healthcare is looking for an Advanced Practice Provider (APP)—an APRN or PA—to manage uncomplicated acute and stable chronic illnesses in a primary care setting. You will perform health assessments, develop diagnostic plans, and coordinate care with a multidisciplinary team.

The role requires certification as a Nurse Practitioner or Physician Assistant and current Illinois licensure. Strong patient communication and collaboration skills are essential in a fast-paced environment.

Qualifications

• Completion of NP or PA program leading to certification.
• Licensed to practice as NP or PA in Illinois.
• Experience in primary care is preferred.

Responsibilities

• Performs comprehensive health assessment through patient/family history, physical examination.

Job Summary:The Advanced Practice Provider (APP) is a primary care Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) who manages uncomplicated acute and stable chronic illness. The APP is responsible for health assessment, early detection of illness, health promotion, disease prevention and management.
