Job summary

SANTA CRUZ HEALTH SERVICES INC is seeking a dedicated Registered Nurse for full-time in-home care. You will provide 1:1 nursing to medically fragile children and young adults, performing skilled nurse visits and coordinating care with families in the patients’ homes.

Experience with Trach/Vent or GJ-tube is preferred; BLS and valid RN license are required. Reliable transportation and ability to travel to clients’ residences are essential.

Qualifications

• RN license and BLS certification are required.
• Trach/Vent and GJ-tube experience is preferred.
• Must have reliable transportation and the ability to travel to patients’ homes.

Responsibilities

• Provide 1:1 home health nursing to medically fragile children and young adults.
• Deliver private duty nursing, skilled nurse visits and associated therapies in patient homes.
• Coordinate care with families and healthcare teams to ensure safe patient outcomes.
