Job summary

Community Health and Wellness Partners in Russells Point, OH is seeking a Patient Care Assistant (PCA) to join our Nursing department on a full-time basis. The role supports a team-based care model across multiple sites, with a Tue-Fri four 10-hour days schedule and occasional Saturday rotation.

As a PCA you will collect data, assist with medications and treatments, complete charting, and coordinate with the Clinical Coordinator and Providers to promote patient outcomes while upholding CHWP’s

Qualifications

• Registered or Licensed Practical Nursing Degree; or Medical Assistant Certification/Licensure.
• 3–5 years of nursing experience in ambulatory health care setting or integrated primary care.

Responsibilities

• Participates as a member of the team-based care model to support patient care goals.
• Provides direct nursing or medical assistant care to patients as assigned.
• Supports care plans to optimize patient outcomes with the team.
• Completes all patient charting and maintains current medical records.
• Attends meetings and continuing medical education to maintain licensure and best practices.
