Qualifications

• High school diploma preferred.

Responsibilities

• Assist with ADLs: bathing, grooming, dressing and personal hygiene; help with mobility and transfers.
• Provide medication reminders and support medication routines.
• Plan and prepare meals adhering to dietary needs and ensure hydration.
• Offer companionship and social engagement; accompany clients to appointments.
• Monitor health status and report changes to the care team; document care provided.
