Job summary

Diversicare is seeking a dedicated Certified Nurse Assistant (C.N.A.) to support residents across our post-acute care facilities. You will assist with transfers, accompany residents to medical visits, and help ensure documentation and prescriptions are correctly managed during and after appointments.

Applicants should hold a valid CNA certification, a valid state driver’s license, and a clean driving and criminal background record.

Qualifications

• Certified Nurse Assistant (C.N.A.) with valid credentials.
• Valid driver’s license and clean driving record.
• Criminal background check cleared and ability to pass state requirements.

Responsibilities

• Ensure resident safety during transport to and from medical visits.
• Accompany residents on visits and remain with them until completion.
• Assist with obtaining documents and prescriptions after appointments.
• Report outcomes and follow-up requirements to the facility team.
