Responsible for coordinating patient transfers, ensuring effective communication between healthcare providers, and maintaining high standards of patient care in a fast-paced call center environment. Facilitates the prompt transfer of patients between facilities and internal transfers, utilizing clinical expertise to assess patient needs and determine appropriate transfer options, ensuring continuity of care. Acts as a central communication source for all components of the patient transfer process, including direct admissions and critical care support. Maintains accurate and timely documentation of pertinent patient information, transfer details and communication with healthcare providers. Advocates for the patients through consideration of patient condition, service requirements and urgency.
Three years of experience as a Registered Nurse in a critical care and/or emergency department is required.
Bachelor of Science in Nursing is required.
