La Clinica de Familia (LCDF) is a federally qualified health center with several locations in Southern New Mexico, serving residents for over 40 years. LCDF is expanding to form a Mobile Crisis Response Team; the Case Manager will support the team and help develop, evaluate, and coordinate treatment plans and recovery services.

• Bachelor’s degree or Associate’s in human services with 2 years in behavioral health or substance use treatment.
• Working unconventional hours: weekends/evening/holiday hour.
• Valid NM driver's license and current automobile insurance; maintain a clean driving record.
• Willing to use personal vehicle for employment when needed.
• Must submit to LCDF required background check, TB screen and drug testing.

Responsibilities

• Provide services to the Mobile Crisis Response Team.
• Assist in developing, evaluating and coordinating treatment plans and recovery services.
• Collaborate within an interdisciplinary team and coordinate care.

Education

Job Summary

With the expansion of LCDF services as a newly designated Certified Behavioral Health Community Center, we are creating a Mobile Crisis Response Team. For this position the Case Manager will provide services to the team. Additional duties include assisting in developing, evaluating and coordinating treatment plans and recovery services.

• Demonstrate competency in case management procedures and allocation of resources and in the characteristics of substance use and levels of severity.

• Adopts a culturally sensitive and non-stigmatizing approach.
• Display familiarity with models of care management.
• Adopt a patient centered approach which emphasizes self-determination and shared decision making.
• Maintains confidentiality and discretion as a rule.
• Ability to work effectively within an interdisciplinary team and communicate effectively with others.
• Familiar with all personnel and company program policies and procedures.
• Ability to multi-task, prioritize, and work under pressure without losing sight of objectives.
• Ability to effectively use Electronic Health Records.
• Must be able to work Tuesday-Saturday from 4pm to 12am.
