Job summary

Amedisys is seeking a Patient Services Coordinator in Bangor, ME to manage scheduling for patient assessments and Oasis visits, coordinating clinicians to maximize efficiency and ensure care is delivered as ordered. This full-time days role focuses on seamless communication with patients, clinicians, and care centers.

Ideal candidates will have a High School diploma or GED and scheduling experience in healthcare, plus administrative exposure.

Qualifications

• High School diploma or GED required.
• One year of scheduling experience in healthcare preferred.
• One year of administrative experience in healthcare preferred.

Responsibilities

• Schedules patient assessment visits including Oasis visits and ensures timely performance.
• Assigns and schedules clinicians in the most efficient geography based on care requirements and clinician capacity.
• Collaborates with field clinicians to ensure visits are completed as ordered and within care center.
• Manages calls from patients and field staff regarding scheduling issues.
• Refer clinical issues to the clinical manager as needed.
• Monitors hospitalized patients and ensures timely follow-up by the team.
• Communicates with patients, clinicians, referral sources and office staff to maintain timely care.
• Runs reports and responds to workflow with appropriate actions.
• Assists with transfers of patients between care centers or hospice services.
• Maintains supply closet and liaises with field for patient supply needs.
• Performs other duties as assigned.
