Pay Range

This range is provided by CenterWell Home Health. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Base pay range: $45,400.00/yr - $61,300.00/yr

Role Overview

The Patient Services Coordinator-LPN is responsible for scheduling visits, communicating with staff and patients, and maintaining care coordination. The role also involves office and staff management tasks.

Key Responsibilities

1. Manage patient schedules and update agent availability.
2. Initiate infection control forms and coordinate reports.
3. Assist during busy times, including field calls and case conferences.
4. Maintain hospitalization logs and send updates to staff.
5. Complete and reschedule visits, including add-ons and supervisory visits.
6. Run scheduling reports and prepare weekly schedules for approval.
7. Verify visit notes and assist with patient transfers.
8. Handle lab reports and participate in patient visits if clinical.

Required Skills and Experience

• Licensed as a Nurse or Vocational Nurse in your state.
• At least 1 year of home health experience.
• Prior review/QI experience and coding certification are preferred.
• Valid driver’s license and insurance.
• Excellent communication, organizational skills, and ability to manage multiple tasks.

Work Details

• 40 hours per week.
• Salary: $45,400 - $61,300 annually, based on location and experience.
