Base pay range

$45,400.00/yr - $61,300.00/yr

The Patient Services Coordinator-LPN is responsible for scheduling visits, communicating with staff, patients, and physicians to maintain care coordination, and assisting with office and staff management.

• Manage patient schedules and reassign as needed.
• Initiate infection control forms and handle related documentation.
• Support during busy times, including receiving calls and assisting with conferences.
• Maintain hospitalization logs and send updates to relevant staff.
• Complete and verify schedules for visits and add-ons.
• Run scheduling reports and prepare weekly schedules for approval.
• Assist with internal patient transfers and handle lab reports if clinical.
• Participate in patient visits and on-call rotations if clinical.

Required Experience/Skills

• Licensed as a Professional Nurse or Vocational Nurse in the practicing state.
• At least 1 year of home health experience.
• Preferred: packet review/QI experience, coding certification.
• Valid driver’s license and insurance.
• Excellent communication, organization, and multi-tasking skills.

Scheduled weekly hours: 40

Pay range: $45,400 - $61,300 per year
