Position Overview

The Patient Services Coordinator, LPN/LVN acts as a vital link between clinical staff and patients, managing scheduling, organization, and healthcare maintenance. The role involves decision-making within established procedures and standards, with some interpretation.

Responsibilities

1. Manage patient schedules, handle sick calls, and reassign patients as needed.
2. Initiate infection control forms and coordinate with HRD.
3. Provide backup support during busy times, including field calls and case conferences.
4. Maintain and update client hospitalization logs.
5. Complete scheduling tasks, including skilled nurse visits and add-ons.
6. Generate and review scheduling reports, prepare weekly schedules, and verify visit notes.
7. Assist with patient transfers and review lab reports if clinical.
8. Participate in patient visits and on-call rotations if clinical.

Required Skills and Experience

• Licensed as a Professional Nurse or Vocational Nurse in the practicing state.
• Minimum of 1 year home health experience.
• Preferred: Packet review/QI experience, coding certification.
• Valid driver’s license and automobile insurance.
• Strong communication, organizational, and multitasking skills.

Additional Details

• Work Hours: 40 hours/week.
• Salary Range: $45,400 - $61,300 annually, based on skills and location.
