Job summary

Enhabit Home Health & Hospice is seeking a dedicated coordinator to schedule patients with branch field clinicians and support seamless care coordination. You will liaise with patients, physicians, referral sources, and caregivers to ensure continuous, high-quality home health services.

The role requires at least a high school diploma, with preferred LPN/LVN licensure or relevant home health/hospice experience in the last two years. Strong tech skills and reliable transportation are essential.

Qualifications

• Must possess a high school diploma or equivalent.
• Must either be a licensed practical or vocational nurse in the state where they practice, with at least one year of clinical healthcare experience; or have at least one year of home health, hospice, or pediatric experience within the last 24 months.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Schedule patients to branch field clinicians.
• Coordinate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.

Education And Experience, Preferred

• Previous experience in home health, hospice, or pediatrics is preferred.
  

Requirements*

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license
