Base pay range

$48,000.00/yr - $65,000.00/yr

General Summary:

The Patient Services Coordinator works with a specified physician practice (Dr. Richard A. Berger) to promote the effective utilization of services and coordination of post-acute care for patients. The Coordinator contributes to the practice's effectiveness by evaluating patients for post-acute needs along the continuum. The Coordinator functions as a liaison between patients, physician practices, clinics, the hospital, and the community, ensuring that patients' continuity of care needs are met. They exemplify the Rush/M.O.R mission, vision, and values and act in accordance with Rush/M.O.R policies and procedures.

Principal Duties and Responsibilities:
  1. Works with physician practice to assess patients' post-acute healthcare needs, monitors patients' progress, and confers with the healthcare team regarding care coordination.
  2. Provides basic assessment of patient's financial and functional support, and creates a plan with the healthcare team to meet the needs of the patient.
  3. Collaborates with the healthcare team and provider to determine the length of stay of the patient and discharge management activities. Promotes cost-effective, quality care services for patients post-acute.
  4. Works with the team to monitor and report potential quality issues.
  5. Utilizes the Case Management information system and appropriate technologies.
  6. Provides services including patient-centered interviews, transition in care screenings, identification of potential post-discharge needs, presentation of patient resource folders, and comprehensive documentation of findings.
  7. Handles all processes related to care transitions, including referrals to external agencies and ongoing communication with external agencies as necessary for patient care.
  8. Ensures compliance with HIPAA and ROI regulations and protocols.
  9. Identifies issues, resolves when able, triages to the appropriate level, and collaborates with the rest of the healthcare team.
  10. Promotes customer satisfaction among patients, families, physicians, external care managers, payers, vendors, and inpatient staff. Actively promotes ICARE principles.
Knowledge, Skills, and Abilities:
  • Bachelor's degree from an accredited university required.
  • Ability to perform tasks independently, prioritize workload, and problem-solve.
  • Knowledge and proficiency with information systems and Windows technology; competency within the CM Information System.
  • Commitment to teamwork, a collaborative approach, and customer service focus. Flexibility to adjust schedule as workload warrants.
  • Ability to travel throughout the Medical Center.
