UCLA Outpatient Clinics is seeking an efficient support professional to aid physicians and patients in assigned primary and/or specialty care areas in Burbank, CA.

You will act as a liaison, help patients access services within the health system, coordinate with referring physicians, and ensure follow-up using EPIC/Care Connect, delivering seamless care coordination. This role requires tact, teamwork, and a patient-centered approach to improve access and satisfaction across clinics.

In this role you will provide direct support to the physicians and patients for assigned primary and/or specialty care physicians.

Major duties include but are not limited to:

• Acting as a liaison for the patients and physicians in the practice.
• Assisting patients in accessing service within the health system.
• Facilitating communication with referring physicians.
• Referring patients to appropriate resources and providing follow-up through telephone and EPIC/Care Connect

Salary Range: $31.88 - $45.66/hourly
