Job summary

Memorial Health System is seeking a patient safety and quality improvement professional to join our team on-site. You will monitor incidents, conduct root-cause analyses, and coordinate cross-functional efforts to reduce safety risks and improve outcomes.

Ideal candidates hold a nursing or healthcare degree, with 3+ years of related experience. Illinois licensure is preferred; CPPS/CPHQ is a plus. We offer a comprehensive benefits package and a collaborative, just-culture work environment.

Qualifications

  • Bachelor’s degree in nursing or healthcare required; 3+ years of healthcare experience.
  • CPPS and/or CPHQ preferred but not required; CPJ culture knowledge a plus.
  • Illinois license preferred but not required; strong collaboration skills.

Responsibilities

  • Monitor, investigate, and analyze patient safety events and near misses.
  • Conduct RCA and FMEA to identify root causes and prevent recurrence.
  • Support initiatives reducing hospital-acquired conditions and errors.
  • Ensure regulatory compliance with Joint Commission, CMS, and state rules.
  • Maintain documentation for audits, surveys, and accreditation reviews.
  • Coordinate patient safety activities across departments and teams.
  • Promote a just, transparent safety culture and continuous improvement.
