Job summary

St. Charles Health System in Bend, Oregon seeks a Patient Safety Assistant to provide constant observation for patients at risk of harming themselves or others, under RN supervision, and to maintain a safe environment.

The PSA documents observations accurately, assists with daily living activities, and follows safety protocols while safeguarding patient dignity. The role requires strong communication, thorough documentation, and a professional, compassionate approach to patients and families

Qualifications

• High school diploma or GED required.
• AHA Basic Life Support for Healthcare Provider certification required before orientation.
• Code Gray or AVADE Training within 3 months of hire.
• Preferred: CNA, EMT, or Medical Assistant licensure; RN student progression acceptable.
• Must be willing to take additional courses as required for the position.

Responsibilities

• Provide continuous monitoring with constant visual observation to ensure patient safety.
• Report changes to nursing staff promptly and participate in safety protocols.
• Assist with activities of daily living as needed.
• Document observations and patient responses accurately in medical records.
• Maintain a therapeutic, respectful approach with patients from diverse backgrounds.
• Travel between St. Charles Hospitals and nursing departments as required.
