Job summary

Gracelight Community Health in Los Angeles seeks a Patient Retention Manager to lead retention, enrollment and attribution strategies. You will analyze membership data across plans and service lines to drive initiatives that increase assigned lives and reduce attrition while protecting coverage under Medi-Cal and Covered California.

You will own the retention plan, build dashboards in Power BI, coordinate with enrollment staff, health plans, and clinical leadership, and train staff on coverage

Qualifications

• Five or more years of experience in managed care, population health, patient access, or healthcare operations.
• FQHC or community health center experience preferred.
• Working knowledge of Medi-Cal, Medicare, and Covered California eligibility, enrollment, and attribution processes.
• Demonstrated ability to analyze large data sets and translate findings for leadership.
• Experience building dashboards and reports in Power BI or comparable analytics tools; advanced Excel.
• Epic experience preferred.
• Ability to translate policy changes into workflows and train staff.
• Personal cell phone available for business use; stipend provided per policy.

Responsibilities

• Own the annual patient retention and membership growth plan with monthly targets.
• Build and maintain the retention dashboard tracking key metrics and performance.
• Lead the Medi-Cal coverage retention program and coordinate outreach around redetermination and work requirements.
• Coordinate with health plans, IPAs, and enrollment staff to reconcile rosters and reduce unassigned members.
• Design patient reactivation and recall workflows and coordinate with site teams on execution.
• Partner with managers and clinical leadership to remove operational causes of attrition and improve access.
