Responsibilities

• Conduct proactive outbound outreach to patients identified through quality reports and registries.
• Educate patients on preventive services, screenings, and follow-up care using approved guidelines.
• Schedule and coordinate appointments, labs, referrals, and screenings to address care gaps.
• Collaborate with clinicians and staff to document and close care gaps in the EHR.
• Maintain accurate documentation of outreach attempts and patient responses in systems.
