Qualifications

• Indiana/HRSA education requirements per state.
• Community Health Worker certification within 90 days of employment.
• Lifestyle Training Coach certification within 1 year of employment.
• Indiana Navigator License within 3 months of employment.
• Application Counselor Certification within 3 months of employment.
• SHIP Certification within 1 year of employment.

Responsibilities

• Educate patients on services and networks and explain program purposes and impact.
• Assist with Medicaid PCP assignments or switches.
• Coordinate intake for new patients and provide social service referrals.
• Assist patients in accessing health-related services and care navigation.
• Help patients resolve health insurance issues and grievances.
• Collaborate with PCP and care team for patient support.
