Job summary

Sun River Health is hiring a full-time Patient Navigator at our Peekskill, NY site. The role supports linking patients to preventive and primary healthcare services, including translation and transportation assistance, and helps navigate referrals with care coordination across departments and external providers.

The position offers a $22.00–$23.73 per hour pay range, a $1,500 sign-on bonus for eligible external applicants, and requires a High School Diploma/GED with 0-2 years of experience;

Qualifications

• Minimum Education: High School Diploma/GED.
• Minimum Work-Related Experience: 0-2 years.
• Bilingual Spanish is preferred.

Responsibilities

• Consult with medical staff to link patients to services through translation, transportation and application assistance.
• Facilitates patient appointments within SRH and outside agencies; refer to SRH providers and outside agencies using preferred provider lists.
• Assist patients in overcoming obstacles to referrals.
• Facilitate patients to appropriate referred agencies; track progress in eCW and alert providers.
• Answer patient questions to clarify instructions, procedures and referral needs.
• Transmit clinical information to specialists for referrals; obtain reports and consent for release of information.
• Review open referrals for upcoming medical appointments and provide to clinical team.
