The role emphasizes advocacy, communication, and collaboration across disciplines. This full-time, hybrid position offers 40 hours weekly with one remote day and four days in offices at 260 Merrimac Street, Newburyport, MA and 1 Park Way, Haverhill, MA.

Qualifications

  • Bachelor's Degree in related field of study required; relevant experience can be considered in lieu of degree.
  • 1+ years of patient facing, community health, medical office or other related experience strongly preferred.
  • Familiarity with electronic health records and case management software.

Responsibilities

  • Advocate for patients and families, ensuring needs are considered in care planning.
  • Liaise between patients, healthcare providers, and other relevant stakeholders.
  • Coordinate patient care services, appointments, and follow-up plans.
  • Educate patients about conditions, treatment options, and self-care strategies.
  • Connect patients with community resources and financial assistance programs.
  • Maintain accurate documentation of patient interactions and referrals.
