Job summary

Center for Family and Child Enrichment in Miami Gardens, FL is seeking a Patient Navigator to support outreach and patient engagement for its Federally Qualified Health Center. This phone-based role educates patients about services, guides onboarding, and connects them with primary care, dental, behavioral health, and specialty care, while ensuring access and confidentiality.

Qualifications

  • High school diploma required.
  • Associate’s degree preferred in Social Work, Public Health, Health Administration, or related field.
  • Minimum 2 years in community health center outreach, case management, or related work preferred.
  • Florida CHW certification preferred, or willingness to obtain within 6 months of hire.
  • Must pass a Level 2 background screening per Florida AHCA requirements.

Responsibilities

  • Conduct outbound calls to patients on capitation rosters to inform them of CFCE assignment.
  • Educate patients on services including primary care, dental, behavioral health, and specialty care.
  • Guide new patients through onboarding and direct them to scheduling.
  • Follow up with patients on the Sliding Fee Discount Program eligibility, applications, and benefits.
  • Identify barriers to accessing care and connect patients with internal or community resources.
  • Screen patients for social determinants of health, including housing, food insecurity, and transportation needs.
  • Coordinate appointments with front office and clinical teams for referrals.
  • Provide exceptional customer service and route inquiries to appropriate departments.
  • Maintain accurate documentation of outreach, referrals, and patient interactions in EHR and call-tracking systems.
  • Ensure patient confidentiality and HIPAA compliance.
  • Participate in team meetings, trainings, and other duties as assigned.

The Patient Navigator is a vital role in delivering high-quality patient engagement and customer service for our Federally Qualified Health Center (FQHC). This phone-based position focuses on both outbound and inbound communication to educate patients about their assignment to CFCE, the comprehensive services we offer, and how to access care.

Some of the Functions Include:
  • Conduct outbound calls to patients listed on capitation rosters to inform them of their assignment to CFCE.

  • Participate in team meetings, training, and other assigned duties as needed.
  • Perform other duties as assigned.
  • High school diploma required; Associate’s degree in Social Work, Public Health, Health Administration, Human Services, or a related field preferred.
  • Minimum of 2 years of experience in a Community Health Center or Federally Qualified Health Center (FQHC) setting, patient outreach, case management, or community health work preferred.
  • Florida Community Health Worker (CHW) Certification preferred, or willingness to obtain within 6 months of hire.
  • Must pass a Level 2 background screening in accordance with Florida Statute and AHCA requirements.

  • Cultural competency and sensitivity when working with diverse patient populations.
  • Knowledge of trauma-informed, non-judgmental communication and behavioral health terminology.
  • Bilingual in English/Spanish or English/Haitian Creole strongly preferred.
  • Familiarity with community health models and sliding fee programs preferred.
