Job summary

Community Health Centers in Orlando seeks a Licensed Clinical Social Worker or Licensed Mental Health Counselor to support patients and families, addressing social, financial, and psychological needs within a team of CHC clinicians using case management skills.

The role requires 2 years in healthcare, bilingual in English/Spanish preferred, LCSW or LMHC license, and BLS or readiness to obtain. Travel to CHC centers may be required; weekday schedule with occasional weekends.

Qualifications

• Bachelor's degree in healthcare or related field.
• Master's Degree in Social Work from a CSWE-accredited school.
• LCSW licensure or eligible for licensure as required.
• Current BLS certification or within 30 days.
• Must obtain FARS/CFARS certification within first 2 weeks.
• 2 years experience in a healthcare setting preferred.

Responsibilities

• Provide counseling to patients and families addressing social, financial, and psychological aspects.
• Collaborate with CHC clinical staff to coordinate care.
• Utilize case management to support treatment decisions and referrals.
• Maintain patient confidentiality and accurate documentation in the EHR.

• Monday: 8am-6pm.
• Tuesday: 8am-7pm.
• Wednesday and Thursday: 8am-5pm.

Job Summary:

The Licensed Clinical Social Worker assists patients, and their families understand a particular illness, work through the emotions of a diagnosis, and provide counseling about the decisions that need to be made. Works in collaboration with CHC clinical staff (doctors, nurses, and allied health professionals) using case management skills to help patients and their families address and resolve the social, financial and psychological problems related to their health condition.

Qualifications.

Education:

• Bachelor's Degree in healthcare or other related field.
• Master's Degree in Social Work from a Council on Social Work Education accredited school of social work.

Certification/Licensure:

• LCSW – License as Clinical Social Worker.
• Current BLS certification required or must be obtained within the first 30-days of employment.
• Must obtain the Functional Assessment Rating Scale (FARS) and Children's Functional Assessment Rating Scale (CFARS) certification within the first 2 weeks of employment.

Experience:

• 2 years previous experience in a healthcare setting is required.
• Experience in an ambulatory health center preferred.
• Requires someone familiar with and sensitive to Hispanic culture, family customs and child rearing practices.
• Familiarity with multicultural groups, previous work experience in a multicultural setting and willingness to enroll language and multicultural coursework to supplement training if necessary.
• Previous supervisory experience preferred.

Special skills:

• Knowledge of EHR software or equivalent, preferred.
• Bilingual in English and Spanish preferred.
• Maintain confidentiality regarding privileged administrative and client information in a professional manner.
• Intermediate knowledge of Microsoft Office, including Word, Excel and Outlook.

Other requirements:

• Able to work flexible hours including weekends as needed.
• May be required to travel to other CHC centers.
